The Patient-Centered Medical Home (PCMH) is an approach to providing comprehensive care for children and adults, in a health care setting that facilitates partnerships between patients and their primary care physicians.

We accept most major insurance plans. Please remember, it is impossible for Troy Internal Medicine to know every insurance plan. We do our best to know what plans are accepted but ultimately it is up to the patient to know their insurance. Click Learn More for more information.

MCIR Immunization Records

You will need to create an account on MiLogin
You will need a piece of government issued ID that you will need to upload for your account. This ID can be US Passport, Driver’s License, or State ID.
Once your account has been created you will be able to get a list of all the vaccinations that you have received in Michigan.
