About Follow My Health
The Follow My Health patient portal is another tool in helping you to stay healthy. It is a secure universal health record meaning, as long as the office or facility you are visiting is participating in Follow My Health you may connect them and access your health records anywhere you have internet connection.
View results online, send your doctor a message, receive appointment reminders and keep track of other useful health information.

FAQ
How do I sign up for the patient portal?
Either set up an account and select our office on www.followmyhealth.com or call the office at 248-267-5000 ext. 3000 to request a Follow My Health invitation. You will receive the invitation by email.
Once I receive an invitation what do I do?
Open the email from Follow My Health. Click on the link that says “Click Here to begin the registration process.” You will be walked through the remaining steps.
What is my Invitation Code?
Your invitation code is your 4-digit year of birth. (Just the year, not the full birthdate).
How do I access my blood work results?
Laboratory results are located under the “My Health” tab under “Results”.
Nothing is there when I go to access my lab results.
If you set up your account on www.followmyhealth.com, please allow up to 3 business days for us to approve your request. Results and physician communication will not be available until your request is approved.
How do I send my doctor a message?
Click the black link at the top your home page that says, “Send a message”.
Can I set up my Follow My Health account with a mobile device?
Yes, you can! The set up is similar to set up on a desk top computer going through the invitation in your email. You will simply need to download the free FollowMyHealth app after set up.
What if I forget my username?
Call the office and request for a recovery email to be sent. At this time, the recovery email can only be sent to the email address used to register for Follow My Health.
